Refund Request Form

Fort Saskatchewan Minor Hockey strives to make every hockey experience a positive one, but from time to time a refund for one of our programs is necessary.

Our Refund Policy is as follows:

All refunds will be assessed a $100.00 fee.

Refund of fees for individuals dropping out of the program shall be rendered upon request to the Office Administrator.

No refunds after November 30th. Refunds do not include the tryout fee. In cases where reasons beyond the control of the participant require them to leave the program, Fort Saskatchewan Minor Hockey may authorize a refund based on the percentage of time left in the regular season upon written request.

Please CLICK here to print a copy of the FSMSA Refund Request form and submit to the office by email to, mail to Fort Saskatchewan Minor Hockey (PO Box 3071, Fort Sask, AB, T8L 2T1) or drop off at the FSMH/FSMSA Office in the JRC Lobby (10013 96 Avenue) during office hours.

Once processed, the refund will be returned in the same manner as to which it was first paid-credit card refund for credit card payments, cheque for cash and cheque transactions. Questions? Please call the FSMH Office at 780-998-1835.
